Overview

Purpose: The Nurse Practitioner Hybrid Role will provide care to the geriatric and/or long-term care resident within the skilled, long term care facility, independent living or assisted living facility.  This care may include both Primary Care as well as Psychiatric Care. This position will work closely with the Behavioral Health/Psychiatric NP case manager of record, ensuring the patient’s care needs are met in-between the visits of the Case Manager.  The Nurse Practitioner (NP) will be under the supervision of the Clinical Team Lead for Psychiatric Nurse Practitioners, the Account Manager, and the Medical Director and/or the Primary Care Physician for the resident and will adhere to all state approved medical and nursing guidelines.

Responsibilities

Essential Job Functions:

 

Performs assessment of the resident.

  • Elicits, records, and interprets the past medical history, family, and social history as it applies to the resident.
  • Performs and records the assessment of the resident for data entry into the EMR, differentiating between normal and abnormal findings. 
  • Provide effective and timely clinical interventions to minimize psychiatric related risks to residents for a variety of presenting issues; including but not limited to, complications from Gradual Dose Reductions, new, worsening or change in behavior or mental status, medication refills or any other urgent needs.
  • Assist with psychiatric crisis intervention needs of the resident as reported by staff.
  • Consults with the Psychiatric NP, as needed, to ensure continuity of care.

 

Plans, implements, and evaluates the management of the resident.

  1. Develops a problem list, impression and plan of care for each resident  
  2. Implements care systems and interventions that reduce patient need for hospitalization.
  3. Completes all EHR charting and other required documentation within 360 Care compliance standards.
  4. Communicates information regarding the resident’s condition to nursing staff, attending physician, patient, family and or responsible party.  Information can include diagnosis, plan of care and prognosis.
  5. Collaborates with other health care team members regarding the provision of care to the resident.
  6. Recognizes medical problems and complications that require further physician evaluation and treatment and refers appropriately to the attending physician and / or specialist.
  7. Completes accurate and timely documentation of assessment and plan in E.H.R. in accordance with Company and Medicare policies.
  8. Actively engages in Company initiatives as defined and requested.
  9. Responsible for prioritizing rounding pattern based on acuity and severity of resident needs.
  10. Proactively identifies gaps in patient care, acute changes in condition and other patient care needs that require medical attention
  11. Is available to participate in meetings with clinic staff that are medically appropriate, with patient specific needs, and a goal to improve patient outcomes and staff communication.
  12. Reviews and responds promptly to coding audits.
  13. Attends training recommended by Clinical Lead.
  14. Response to billing questions promptly.
  15. Assists with training new providers.
  16. May be required to provide on-call coverage, as required by operational and/or clinical needs.

 

 

Performs and assumes responsibility for administrative functions pertinent to the NP’s individual practice.

 

  1. Learns and performs functions of Company EMR software on a timely basis.
  2. Utilizes Company services for human resource related needs including time off and timesheet approval.
  3. Meets with collaborating physician in timely manner according to state rules / regulations and completes all documentation in regard to this in timely manner.
  4. Is able to effectively utilize a multi-function machine (scan, copy, and fax), laptop computer and other devices as needed for proper and timely implementation of practice.
  5. Is able to have a home-based office with storage of supplies for the primary care practice as needed.
  6. Works in partnership with the Practice Manager (PM) and other administrative support staff as necessary.
  7. Follows all Company policies and operations protocols.
  8. Completes expense reports on a timely basis.
  9. Returns all Credentialing and other paperwork that is required of the Nurse Practitioner by the corporate staff that day or within the timeframe requested

 

Assumes responsibility for own professional growth through continuing education.

  1. Continuing education seminars are to be approved through the NP’s immediate supervisor and VP of Clinical Programs
  2. The NP is to be responsible to obtain all necessary CEU’s in order to keep licensure and certification current according to his or her state regulations and the regulations of the appropriate credentialing body as well as providing electronic copies for administrative credentialing staff at Company. Copies of all completed CEUs should be sent to the attention of the Provider Relations Analyst at the Company Corporate office.
  3. State certification and credentialing must be maintained. 
  4. The NP is responsible for ensuring that licenses and credentials are current and providing all copies of each as requested.

 

Participation in Healthcare Effectiveness / QI Programs:

  1. All NPs must be aware of their respective state rules and regulations which govern their practice.
  2. Each NP is to ensure that he or she follows all state requirements regarding the oversight/supervision of his or her practice.
  3. All NPs must participate in MIPS program and initiatives as defined by Company

 

  • Actively supports and complies with all components of the compliance program, including, but not limited to, completion of training and reporting of suspected violations of law and Company policy.
  • Maintains confidentiality of all information; abides with HIPAA and PHI guidelines at all times.
  • To work remotely, it is expected you will have a reliable, nonpublic high-speed internet connection with sufficient bandwidth to participate in all work-related activities.  Any interruptions in service should be immediately reported to your manager.  
  • Reacts positively to change and performs other duties as assigned.
